This form MUST be filled out by a PARENT/ Guardian in order for our Juniors and Seniors to leave campus at lunch. 

If you need a physical form or if you have any questions please see Megan at the attendance office.

You can pick up your sticker with Megan at attendance once the form is filled out and verified.
